Three Dogs Training Liability Waiver

As a participant in any class or seminar run by or on behalf of Three Dogs Training, I agree that I am solely responsible for the care and control of my animal while in class or seminar and on the premises where the class or seminar is being held as well as on any portion thereof, including but not limited to any outside area or other area adjacent to or surrounding those premises. I hereby release, indemnify and hold harmless Three Dogs Training, Lisa J. Edwards, H.E.A.R.T. Programs, The First Presbyterian Church of Mahopac Falls, Green Chimneys, and the Patterson Recreation Center and all of their respective officials, directors, officers, shareholders, members, employees, agents, representatives, administrators, insurers, heirs, executors, successors and assigns from and against any and all liability, damages, costs, losses, expenses and judgments arising out of or in connection with any incident and/or injury to any animal or person (including me and my animal) which occurs: (a) during any class or seminar run by or on behalf of Three Dogs Training; and/or (b) while on the premises or any portion thereof where any such class is held, including but not limited to any outside area or other area adjacent to or surrounding those premises. I agree that I will immediately report any such incident and/or injury to an instructor of Three Dogs Training. This release, indemnity, and hold harmless may not be altered, modified or changed, except by writing signed by me and a representative of Three Dogs Training.*
